In the memo, include your occupation, employer, and employer city (This information is required in Oregon for campaign finance reporting!)
Mail your check to:

Please note that all the information in the form, including employment information, is legally required in Oregon for campaign finance reporting purposes.

Also, the first $50 (or $100 if filing jointly) of your contribution to the Oregon Rebate may qualify for the Oregon Political Tax Credit (ORS 316.102).
